The sierra leone government introduced a cost recovery system for drugs in the 80s. the scheme was implemented largely successful until the break out of the civil war in the early 90s. after the war, the cost recovery system was re-introduced in 2006, but failed to recover enough costs to make it sustainable. in 2010, a free health care initiative was introduced, with its own separate drug supply system, largely donor financed. finally, in 2013, the cost recovery system was stopped again, due to recovery rates of only 5%.
the ministry of health and sanitation is looking at options to re-introduce a cost recovery system, to provide affordable quality drugs to the population not covered under the free health care initiative. the public hospitals and health facilities in sierra leone are regularly experiencing stock outs of drugs and medical supplies. the situation is exacerbated by the fact that there has not been any central procurement for cost recovery drugs since 2013, and the funding available for free health care drugs is only covering 20% of the total cost need per year. distribution of drugs is supposed to happen on a quarterly basis, but currently happens on an ad hoc basis, about twice a year. hospitals therefore either run their own drug revolving funds in collaboration with private pharmacies, or the nurses and health practitioners are selling out of their pockets to patients. due to the centralized human resource management system, it is not possible to discipline those health practitioners accordingly. |
